情绪化进食是一种消极的情绪应对方式,会给个体生理和心理层面带来一定的负面影响,若未得到充分的干预,有可能发展成为进食障碍或肥胖症,给个体带来更大的健康风险。实证研究表明,辩证行为疗法针对情绪的干预模型有着独特的优势,是干预情绪化进食的首选方案。 本研究使用辩证行为疗法(Dialectical Behavior Therapy, DBT)干预一例情绪化进食的来访者,来访者为一名23岁的女性,是北京某985高校大四学生。整个干预过程包含十二次咨询。咨询过程主要分为正念进食觉知训练、情绪调节技能训练、痛苦承受技能训练三个模块。初评过程及第2次咨询以筛选、评估进食症状、收集来访者基本信息为主,第3-5次咨询进行正念进食觉知训练,第6-8次咨询学习情绪调节技能,第9至11次咨询学习痛苦承受技能,第12次咨询进行结案,与来访者讨论预防复发的应对计划,并就来访者未来可能的成长点进行讨论。 本研究使用“抑郁焦虑压力问卷(Depression, Anxiety and Stress Scale, DASS-21)”、“情绪化进食量表中文版(Emotional Eating Scale, EES)”、“自尊量表(Self-Esteem Scale, SES)”、“简易应对方式问卷(Simplified Coping Style Questionnaire, SCSQ)”、“人际关系综合诊断量表(Interpersonal Relationship Integrative Diagnostic Scale, IRIDS)”对咨询效果进行评估,前后测对比发现,来访者情绪化进食症状得到明显下降,抑郁、焦虑、压力水平回归正常,自尊水平有所上升,对情绪的积极应对增高、消极应对方式显著下降,人际关系困扰得到改善。经过质性分析,发现经过咨询来访者饮食恢复正常化,社会功能恢复较好,人际困扰减少,自我接纳程度提高。 研究证明辩证行为疗法针对有情绪化进食困扰的来访者具有一定可行性。 |

Emotional eating is a negative emotion coping strategy that has negative impact on the individual both physically and psychologically. If not fully intervened, it may develop into an eating disorder or obesity, increasing health risks to the individual. Empirical studies have shown that Dialectical Behavioral Therapy (DBT) has a unique advantage in emotion intervention and is the preferred option for intervention in emotional eating. This study used Dialectical Behavior Therapy (DBT) to treat a client with emotional eating behavior The client is a 23 years old female studying in a university in Beijing. The intervention included 12 sessions, which were divided into three modules: mindful eating awareness training, emotional regulation skills training,and distress tolerance skills training. The initial and the second sessions were focused on evaluating the eating symptoms and collecting the basic information of the client. The mindful eating awareness training were conducted in the 3-5th counseling sessions, the emotional regulation skills training in the 6-8th sessions, and the pain tolerance skills training in the 9-11th sessions. The intervention was completed in the 12th session where a recurrence prevention and response plan was discussed. This research evaluated the effectiveness by referring to “Depression, Anxiety and Stress Scale(DASS-21)”, “Emotional Eating Scale Chinese version(EES)”, “Self-Esteem Scale” (SES), “Simplified coping style questionnaire(SCSQ)”, “Interpersonal Relationship Integrative Diagnostic Scale(IRIDS)”. The contrast of the client’s behaviors before and after the intervention shows that the emotional eating symptoms were significantly reduced. Meanwhile, the levels of depression, anxiety and stress returned to normal, and the self-esteem levels and positive responses to emotions increased, while negative trait coping style decreased significantly, and interpersonal problems were improved. After qualitative analysis, it was found that after the intervention, the diet of the client returned to normal, social function was well recovered, interpersonal distress was reduced, and self-acceptance increased. The study has shown that dialectical behavioral therapy is feasible for clients with emotional eating .
