研究目的:以太极康复方案为研究手段对高血压患者进行为期24周的RCT健康干预,探究太极康复方案干预前后高血压患者睡眠质量及血清褪黑素水平的变化,尝试在前人研究基础上进一步揭示太极康复方案对慢性病防治的机制研究,为太极拳临床干预提供循证依据。研究方法:采用2时间(干预前、干预后)×3组别(实验组、对照组、空白对照组)的混合实验设计,两种干预方案为自变量,PSQI量表总分及各子维度得分、血清褪黑素(MELT)为因变量。对符合招募条件并纳入的被试按照1:1:1随机分为实验组(太极康复方案)、对照组(常规运动康复方案)和空白对照组,进行为期24周的运动干预。干预期实验组与对照组严格按照实验团队要求执行运动康复方案,并在前后两次测量匹兹堡睡眠量表(PSQI)、血清褪黑素水平。研究结果:太极康复方案与常规运动康复方案对高血压患者干预后睡眠质量总体水平均有显著差异,太极康复方案效果优于常规运动康复方案,空白对照组无显著变化;太极康复方案组在主观睡眠质量维度、入睡时间维度、睡眠时间维度、睡眠障碍维度、催眠药物维度、日间功能障碍维度得分均显著降低,在睡眠效率维度没有显著差异;常规运动康复方案组在主观睡眠质量维度、睡眠障碍维度得分显著降低,在入睡时间维度、睡眠时间维度、睡眠效率维度、催眠药物维度、日间功能障碍维度均无显著差异,但得分有下降趋势;空白对照组各维度均无显著变化。太极康复方案干预显著提升了高血压患者的血清褪黑素水平,常规运动康复方案对血清褪黑素水平的影响不具有显著性,空白对照组血清褪黑素水平显著下降。研究结论:(1)24周太极康复方案对高血压患者的睡眠质量有显著改善效果,在主观睡眠质量维度、入睡时间维度、睡眠时间维度、睡眠障碍维度、催眠药物维度、日间功能障碍维度均能有效改善,且改善效果优于常规运动康复方案。(2)24周太极康复方案对高血压患者血清褪黑素水平有显著提升效果,主观问卷评价结果与生化指标结果一致,太极拳能够促进褪黑素的分泌,改善睡眠质量。 |

Research Objective: The study aims to investigate the changes in sleep quality and serum melatonin levels in hypertensive patients before and after a 24-week randomized controlled trial (RCT) health intervention using a Tai Chi rehabilitation program. The study seeks to further elucidate the mechanisms of Tai Chi rehabilitation program in the prevention and treatment of chronic diseases, based on previous research, and to provide evidence-based support for the clinical application of Tai Chi.Research Method: A mixed experimental design with two time points (pre- and post-intervention) and three groups (experimental group, control group, and blank control group) was adopted. The two intervention plans served as independent variables, while the total score and sub-dimension scores of the Pittsburgh Sleep Quality Index (PSQI), melatonin (MELT) were dependent variables. Participants who met the recruitment criteria were randomly divided into an experimental group (Tai Chi rehabilitation program), a control group (conventional exercise rehabilitation program), and a blank control group at a 1:1:1 ratio. They underwent a 24-week exercise intervention. Both the experimental and control groups strictly followed the exercise rehabilitation program as required by the research team and were measured twice for the PSQI, serum melatonin.Research Findings: Both the Tai Chi rehabilitation program and the conventional exercise rehabilitation program showed significant differences in improving the overall sleep quality level of hypertensive patients after intervention, with the Tai Chi rehabilitation program being more effective than the conventional exercise rehabilitation program, while the control group showed no significant changes. The Tai Chi rehabilitation program group experienced a significant reduction in scores for subjective sleep quality, sleep onset, sleep duration, sleep disorders, hypnotic medication, and daytime dysfunction, with no significant difference in sleep efficiency. The conventional exercise rehabilitation program group saw a significant reduction in scores for subjective sleep quality and sleep disorders. However, there were no significant differences in sleep onset, sleep duration, sleep efficiency, hypnotic medication, and daytime dysfunction, although there was a downward trend in scores. The control group showed no significant changes in any dimension.The Tai Chi rehabilitation program intervention significantly increased the levels of serum melatonin, while the conventional exercise rehabilitation program did not have a significant impact on serum melatonin levels. The control group experienced a significant decrease in serum melatonin and related indicators.Research Conclusions:(1) A 24-week Tai Chi rehabilitation program has a significant improvement effect on the sleep quality of hypertensive patients, effectively improving subjective sleep quality, sleep onset, sleep duration, sleep disorders, hypnotic medication, and daytime dysfunction, with better results than the conventional exercise rehabilitation program.(2) The 24-week Tai Chi rehabilitation program significantly enhances the levels of serum melatonin. The results from subjective questionnaire evaluations are consistent with objective biochemical indicators, suggesting that Tai Chi can promote the secretion of melatonin and improve sleep quality. |
