随着新医改政策的出台,基层推动落实家庭医生签约服务,对于基层医务人员来说有更高的要求,日常工作在诊疗疾病的基础上,增加了对居民健康管理等方面的工作。工作内容增多、工作任务加重,更加强调以人为中心,工作理念与以往有所不同,基本医疗和公共卫生服务的提供在工作中同等重要,这使得所承受的角色压力越来越大,如果任由基层医务人员角色压力发展,会对员工个人与组织的发展产生危害,产生职业倦怠、离职意愿等,不利于基层医疗卫生行业的发展及新医改的顺利推进。

本研究通过横断面研究的方法,对廊坊市基层医务人员进行问卷调查,包括研究对象的基本人口学信息(性别、年龄、婚姻状况、人事关系等)、角色压力、职业倦怠情况,完成后当场核查并收回。通过数据分析,得出以下结果:职业倦怠及其维度在被调查者的人口统计学信息变量上除职员类型外均不具有显著差异;角色压力及其三维度在性别、平均每周工作天数上不具有显著差异;角色压力在职员类型上具有显著差异;角色冲突在婚姻状态、学历、岗位类别、人事关系、职员类型上具有显著差异;角色模糊在婚姻状态、职员类型上具有显著差异;角色负载在年龄、婚姻状态、工作年限上具有显著差异;基层医 务人员具有一定程度的角色压力和职业倦怠的状况,需受到有关部门注意;基层医务人员角色压力与职业倦怠具有正相关关系,且角色压力的三个维度,即角色冲突、角色模糊、角色负载,也与职业倦怠存在正相关关系。因此,角色压力及其各维度均对职业倦怠具有显著影响,角色压力及其各维度是基层医务人员职业倦怠的重要影响因素。 基于上述分析,本研究则探究基层医务人员在新医改后的角色压力、职业倦怠情况,以及角色压力对职业倦怠是否有影响、存在怎样的关系,得出相关结论,从政策、社会、组织、个人四个层面提出对策建议,为此后基层医务工作优化开展的参考依据。 |

With the introduction of the new medical reform policy, primary implementation of family doctor contracting services has higher requirements for primary healthcare workers. On the basis of diagnosis and treatment of diseases, daily work has increased work on residents' health management. Increased work content, heavier work tasks, and more emphasis on people centeredness. The work concept is different from the past. The provision of basic medical and public health services is equally important in the work, which makes the role of the role under increasing pressure. The development of pressure from the role of primary healthcare workers will harm the development of employees and organizations, and cause job burnout and willingness to resign, which is not conducive to the development of the primary medical and health industry and the smooth progress of new medical reforms.

This study conducted a questionnaire survey on primary healthcare workers in Langfang City through a cross-sectional study, including the basic demographic information (gender, age, marital status, personnel relations, etc.), role pressure, and job burnout of the research subjects. After the questionnaire survey is completed, check and withdraw. Through data analysis, the following results are obtained: job burnout and its dimensions do not have significant differences in the respondents’ demographic information variables except for the type of employee; role pressure and its three-dimensionality are in terms of gender and average number of working days per week there are no significant differences; role pressure has significant differences in employee types; role conflicts have significant differences in marital status, education, job category, personnel relations, and employee types; role blurring has significant differences in marital status and employee types; roles load has significant differences in age, marital status, and working years; primary healthcare workers have a certain degree of role pressure and job burnout, which needs to be paid attention to by relevant departments; role pressure of primary healthcare workers has a positive correlation with occupational burnout, and the three dimensions of role stress, namely role conflict, role ambiguity, and role load, also have a positive correlation with job burnout. Therefore, role pressure and its various dimensions have a significant impact on job burnout. Role pressure and its various dimensions are important factors influencing the burnout of primary healthcare workers. Based on the above analysis, this study explores the role pressure, job burnout of the primary healthcare workers after the new medical reform, and whether the role pressure has any effect on job burnout and what kind of relationship exists, and draws relevant conclusions from policies, society, organizations, Individuals put forward countermeasures and suggestions at four levels, and this is the reference basis for the optimization of primary medical work. |
