回顾性研究分析 2014 年 12 月-2017 年 9 月间中国人民解放军总医院第七医学中心NICU 患者资料,分析血清中 BG 值和血小板参数的差异性与相关性,以及抗真菌治疗前后二者参数的变化,以此探究血小板指标联合 G 实验是否可用于新生儿 IFI 早期临床鉴别诊断。结果显示,与未感染者组相比,IFI 组婴儿的 PLT,PCT 和 PDW 值显著降低,BG值,MPV 和 P-LCR 值显著升高。与细菌组相比,IFI 组婴儿的 PLT 和 PCT 值也明显较低,BG 值,MPV 和 P-LCR 值显著较高,PDW 值较为复杂,但同样具有显著差异性。IFI 组BG 值与血小板参数存在相关性,其中,BG 值与 PLT、PCT、PDW 呈负相关,与 MPV、P-LCR 呈正相关。抗真菌治疗后,IFI 组 PLT,PCT 和 PDW 上升,MPV 降低,P-LCR 变化不明显。因此,在新生儿临床检测中,BG 值与血小板参数变化可用于提示患者受致病菌侵害,二者联合检测与动态监测有助于 IFI 早期发现、诊断与治疗。

A retrospective study analyzed the data of NICU patients in the Seventh Medical Center of the Chinese People's Liberation Army General Hospital between December 2014 and September 2017, and analyzed the differences and correlations of serum BG values and platelet parameters, as well as the two parameters before and after antifungal treatment. In order to explore whether platelet index combined with G test can be used for early clinical differential diagnosis of neonatal IFI. The results showed that compared with the uninfected group, the IFI group infants had significantly lower PLT, PCT and PDW values, and significantly higher BG values, MPV and P-LCR values. Compared with the bacterial group, the infants in the IFI group also had significantly lower PLT and PCT values, significantly higher BG values, MPV and P-LCR values, and more complex but equally significant differences in PDW values. There was a correlation between BG value and platelet parameters in IFI group. Among them, BG value was negatively correlated with PLT, PCT and PDW, and positively correlated with MPV and P-LCR. After antifungal treatment, PLT, PCT and PDW increased, MPV decreased, and P-LCR did not change significantly in IFI group. Therefore, in the clinical detection of neonates, the changes of BG value and platelet parameters can be used to indicate that the patient is affected by pathogenic bacteria, and the combined detection and dynamic monitoring of the two are helpful for the early detection, diagnosis and treatment of IFI.
