情绪性进食(Emotional Eating)是个体在负性情绪下的过度进食反应,表现为个体大量进食并倾向于选择限制性的食物(例如,高脂肪、高能量的食物)。以往研究认为,情绪性进食是一种失败的情绪调节策略,个体在压力情境下会产生对压力的失控感以及消极的自我意识,个体会回避这种消极自我意识以及其引发的负性情绪,而产生大量进食的行为。这种进食行为的维持会阻塞个体有效应对压力和负性情绪,形成封闭的恶性循环。因此,情绪性进食者需要学会有效调节情绪,重建对压力情境的认识。 本文报告了针对1例情绪性进食来访者进行的10次面对面认知行为取向的个体心理咨询。该来访者在以学业为主的压力情境,由于消极的自我认识和与他人比较而产生的焦虑、沮丧情绪,产生频繁的情绪性进食,具体表现为每周产生2-3次在焦虑、沮丧情绪下的大量进食行为。来访者报名参与研究,并期待通过咨询改善情绪性进食,正确应对压力。 本个案进行了四阶段,共10次的咨询工作。第1—3次咨询为咨询初始阶段,咨询师和来访者建立工作同盟,确定问题清单与咨询目标,引导来访者对情绪性进食进行监控记录,理解情绪性进食的行为模式。第4—7次咨询为咨询干预阶段,咨询师和来访者通过行为功能分析表分析和讨论来访者的情绪调节策略,讨论“转移注意力”、“认知重建”、“与朋友倾诉情绪”等情绪调节策略,增加来访者的情绪调节策略,减少情绪性进食。第8—9次咨询为咨询巩固阶段,咨询师和来访者讨论中间信念,增加来访者对于负性情绪的接纳以及对情绪调节的效能感,从信念层面出发巩固来访者的行为改变。第10次咨询为咨询结束阶段,咨询师和来访者对咨询进行回顾和总结,结束整个心理咨询。 咨询师在咨询过程中监控来访者每周情绪性进食的发生次数,并通过荷兰进食问卷—情绪性进食分问卷、情绪调节困难量表、贝克抑郁量表和贝克焦虑量表从进食行为倾向、情绪调节困难以及情绪三个角度进行咨询效果的评估。来访者经过10次咨询之后,情绪性进食基本消失,荷兰进食问卷—情绪性进食分问卷、情绪调节困难量表等量表得分均得到显著下降。另外,从来访者反馈和咨询师观察来看,来访者对负性情绪的接纳程度提高,发展掌握了“认知重建”等多种相对进食行为而言更具适应性的情绪调节策略,在负性情绪调节上具有了更多的主动性和选择性。 |

Emotional Eating is an overeating response in negative moods, characterized by Eating in large quantities and a tendency to choose restricted foods (E. G. , foods high in fat and energy) . Previous studies have suggested that emotional eating behavior is a kind of failed emotion regulation strategy, and individuals may have a sense of loss of control and negative self-consciousness in stressful situations, the individual will avoid this negative self-consciousness and the negative emotion which it causes, but produces the massive eating behavior, and can block the individual to deal with the pressure and the negative emotion effectively. Therefore, emotional eaters need to learn to regulate their emotions effectively and use cognitive reappraisale in stressful situations. The content of this case report is to conduct 10 face-to-face individual psychological counseling on cognitive behavioral for a client with emotional eating behavior. The client had frequent emotional eating behavior due to her inability to cope with academic stress, anxiety and depression which from her negative self-consciousness and comparison with others. The client enrolled in the study and hoped to control the eating behavior and coping with stress. This case has carried on 10 sessions. The 1-3 session was the initial stage of counseling. The counselor and the client established work alliances, defined problem lists and counseling goals, monitored emotional eating behavior, and try to understood the pattern of eating behavior. The 4-7 session was a counseling intervention phase, in which the counselor and the client discussed the strategies of emotion regulation, as "shifting attention" , "cognitive reappraisale" , "talking to friends about emotions" , to increase clients' emotional regulation strategies and reduce emotional eating behaviors. During the 8-9 session, the counselor and the client discussed the beliefs, increased the client's acceptance of negative emotions and the efficacy of emotion regulation, and consolidated the client's behavior change. The 10th consultation is the end of the consultation, the consultant and the clients summarized the whole session. During counseling, the counselor monitors the number of emotional eating behaviors that occur weekly, the results were evaluated by using the Dutch food intake questionnaire, the Difficulties of Emotion Regulation scale, the Beck Depression Inventory and the Beck anxiety Inventory in the eating behavior, the emotion regulation difficulty and the emotion. After 10 sessions, the emotional eating behavior basically disappeared, the scores all decreased significantly. In addition, according to the feedback from the client and the evaluation from the counselors, the emotional eating behavior of the client was significantly reduced, and they mastered various emotional regulation strategies such as "cognitive reappraisale" , the difficulties of "unacceptance" and "lack of strategies" in emotion regulation were significantly alleviated. The client was able to have more choices to regulate and deal with their emotions and stress. |
