抑郁作为新时代大学校园中流行最广泛的情绪问题,呈现出逐年增长的趋势。由于大学生的身心发展还未成熟,缺乏自主调节能力和应对技巧,加上面临环境适应、学业压力、人际交往等问题,极易出现强烈的内外在冲突,从而导致抑郁等消极情绪的出现。长期存在的抑郁情绪将会使大学生的学业成绩下降、社会交往困难、缺乏活力等,进而出现自责、无望和无价值感,严重危害到大学生学业成就和心身健康的发展,甚至会出现自伤自杀等伤害自身或他人的高风险事件。因此,本研究针对大学生抑郁进行干预具有非常实际的治疗意义和临床借鉴价值。

本研究采用定性个案研究,运用观察调查法与临床咨询实践设计旨在补充和完善情绪聚焦疗法(Emotion-Focused Therapy, EFT)干预大学生抑郁的理论和实践。EFT理论假设抑郁来访者最初表现出未分化的适应不良情绪,但随着咨询进展,逐渐能够区分识别出潜在的核心痛苦情绪,如羞愧、悲伤、恐惧和未满足的安全和依恋需求。EFT是一种基于共情性的治疗工作联盟,通过合作性的治疗过程,来访者最终能够改善问题性的情绪图式,获得新的情绪和应对经验,创造建设性的行动意义,进而提高心理弹性和生命质量。

本研究运用EFT对一名符合轻度抑郁症标准的大学女生进行12次的个体咨询,以痛苦情绪处理的状态转换模型为基础进行定性分析,考察来访者情绪意义的唤起、表达和调节功能的变化。运用贝克抑郁自评量表第2版(Beck Depression Inventory-II, BDI-II)、情绪调节问卷(Emotion Regulation Questionnaire, ERQ)、积极与消极情绪问卷(The Positive and Negative Affect Scales, PANAS)、效果问卷-45Outcome Questionnaire-45, OQ-45)进行前测、中测和后测,评估咨询进展及来访者的情绪转化情况。按照关系的建立与促进觉察、激活和探索、生成新的情绪经验与创造新的叙事意义三个阶段推进咨询进展,来访者不仅恢复到正常情绪,增强了自尊和自我功能,干预效果也取得明显地临床治疗价值,超出预期的咨询目标。2个月后实施效果追踪访谈和问卷调查,结果显示来访者贝克抑郁量表分数为0,即无抑郁情绪。此外,来访者表达在人际功能和家庭互动上发展出建设性行为和有效的沟通策略,对于亲密关系的互动也有了新的体验和理解,证实EFT对于大学生抑郁干预的有效性和适用性,进一步丰富和完善了EFT理论的临床运用和咨询实践。

Depression, which has shown a trend of increasing year by year, is the most common is one of the most common emotional problems on college campuses in the new era. University students are not yet mature in their physical and mental development, lack of self-regulation ability and coping skills, suffer from environmental adaptation, academic pressure and interpersonal communication, are prone to strong internal and external conflicts and negative emotions such as depression. Long-term depression will make university students' academic performance decline, social communication difficulties, lack of vitality and so on, which will lead to self-reproach, hopelessness and worthlessness, which seriously harms college students' academic achievement and development of mental and physical health, and even the occurrence of self-injury suicide and other high-risk events that hurt themselves or others. Therefore, it is of great practical therapeutic significance and clinical reference value to intervene depression in university students in this study.

This research adopts qualitative case study, observational investigation method and clinical consulting practice designwhich is to improve and perfect the theory and practice of Emotion-focused Therapy (EFT) in university students' depression. EFT theorizes that depressed clients initially exhibit undifferentiated maladaptive emotions, but as counseling progresses, they gradually become able to distinguish and identify underlying core painful emotions, such as shame, sadness, fear, and unmet security and attachment needs.

In this study, EFT was used to conduct 12 individual counseling sessions for a female university student who met the criteria of mild depression. The qualitative analysis is based on the state transition model of pain emotional processing, which was conducted to investigate the changes of the client's emotional significance expression, arousal and regulatory function. Beck Depression Inventory-II (BDI-II), Emotion Regulation Questionnaire (ERQ), The Positive and Negative Affect Scales (PANAS), and Outcome Questions-45 (OQ-45) were used to conduct pre-test, mid-test and post-test to evaluate the progress of counseling and the client's emotional transformation. According to the three stages of relationship establishing and promoting awareness, activation and exploration, generating new emotional experience and creating new narrative meaning to promote the counseling progress, The client returned to normal mood, which not only enhanced self-esteem and self-function, but also achieved obvious clinical therapeutic value and exceeding the expected counseling goal. Two follow-up interviews and questionnaires were conducted 2 months later, and the results showed that the beck depression scale score of the client was 0, that is, no depression. Moreover, the client expressed that she developed constructive behaviors and effective communication strategies in interpersonal functions and family interactions, and had new experience and understanding of intimate relationship interaction, which confirmed the effectiveness and applicability of EFT in university students' depression intervention, and further enriched and improved the clinical application and counseling practice of EFT theory.

 本研究者从2019年6月起成为北京师范大学心理健康服务中心的实习咨询师,截止到2020年05月,累计临床咨询时长277.5个小时。服务接待来访者数量22个,及时识别并处理具有高风险危机个案2个,联合带领新婚父母角色适应团体辅导与咨询小组1组。咨询类型涉及家庭咨询2个,婚恋咨询2个,儿童咨询1个,成年个体咨询17个。此外,本研究者于2020年01月27日成为北京师范大学心理学部组织的新冠肺炎疫情网络心理辅导工作者,截止到2020年05月,累计临床辅导时长33个小时左右,其中服务个案类型青少年8次,海外留学生8次,成人个体17次,接受团体督导45个小时左右。 研究者在北京大学第六医院、北京安定医院累计见习时长40个小时,在北京师范大学心理健康服务中心及北京师范大学婚姻家庭研究与咨询中心累计见习时长279个小时。此外,研究者从2018年5月至2020年5月在北京安定医院特需门诊和专家门诊跟诊见习,临床咨询实践期间接受中国心理学会注册督导师专业督导时长累计134.5个小时,临床实践累计总时长731个小时,除了必备的专业技能培训之外,接受危机干预培训48课时,职业伦理培训56课时,具备一定的临床实践经验。 研究者的导师在我们入学以来,在每两周一次的临床实务例会上,通过EFT视频教学分析与案例督导的方式,耐心教授与指导我们,使我们对EFT咨询技术的使用有了更深入具体的了解,丰富了临床治疗的干预技巧,增强了我们对EFT咨询技术运用的信心。
