广泛性焦虑障碍是指个体对一系列的活动或事件过分的担心和焦虑,个体很难控制这种担心,无法聚焦于当前的工作或任务。这种障碍困扰着全球1.83亿人。 目前,多数研究倾向于支持使用认知行为疗法治疗广泛性焦虑障碍,但也有研究显示认知行为疗法在降低焦虑症状方面的效果有限,新兴研究表明精神动力疗法同认知行为疗法同样有效。本文以文献综述和案例呈现的方式,介绍了运用精神动力疗法对一名广泛性焦虑障碍伴躯体症状的来访者进行干预的过程。整个咨询过程中,研究者遵循精神动力疗法的四个步骤开展工作:建立必要的咨访关系;鼓励来访者通过自由联想讨论问题;缓解冲突并促使行为改变,获得新的应对方式;巩固改变。从内驱力理论、自我心理学、自体心理学和客体关系理论四个角度对来访者进行动力学倾听,了解来访者的以生物学为基础的本能(力比多或攻击性),觉察来访者为了适应和调节内驱力所采取的防御机制,发现来访者的自尊和自我价值调节,总结来访者由过去重要人物的记忆构建的愿望、行为、感知和意义在人际关系背景中的发展,从个体发展水平、防御机制、客体关系、自体自尊四个方面对来访者进行个案概念化,真诚对待来访者,无条件积极关注来访者的情绪、身体感受以及心理感受,使用苏格拉底式的提问、自由联想、此时此地、情感反应等咨询技术与来访者共同探讨当前症状的根源,并找到应对焦虑伴躯体症状的方法,有效改善了来访者的焦虑情绪。 本文的创新之处,实证证明精神动力疗法可有效改善来访者焦虑情绪,从个案报告的角度为精神动力疗法的有效性提供了有力的个案证明,同时,实例聚焦反移情,丰富了投射性认同的案例库。文中个案的咨询从2017年10月底开始,至2018年5月底结束,共包含16次正式会谈。根据进展情况,研究者将整个咨询过程分为探索阶段(1-2次)、领悟阶段(3-6次)、巩固阶段(7-11次)、结束阶段(12-16次)。经过16次的咨询,四次施测数据表明来访者在贝克焦虑量表上的得分,有显著的下降并处于正常值的范畴,来访者采用了更加成熟、适应性更强的防御机制。这些都证明了这套干预方案的有效性,但是,个案报告并不具备普遍意义,所以研究者会在以后的工作中进行更多的理论探索与干预研究,希望对广泛性焦虑障碍的干预工作有所帮助。

The essential feature of generalized anxiety disorder is excessive anxiety and worry about a number of events or activities, and the individual finds it difficult to control the worry and to keep worrisome thoughts from interfering with attention to tasks at hand. At present, most studies tend to support that cognitive behavioral therapy is widely used in the treatment of generalized anxiety disorders. However, some studies have shown that cognitive behavioral therapy has limited effect in reducing anxiety symptoms. New studies show that psychodynamic therapy is as effective as cognitive behavioral therapy. Based on literature review, this study presents the whole process of a client suffering from social anxiety disorder with somatic symptoms through the intervention of psychodynamic therapy. Throughout the consultation process, The author followed the four steps of psychodynamic therapy: a) building the necessary alliance and encouraging motivation to change; (b) helping the patient discover various problems (personality disorders are not as easily recognized by the patient as are anxiety, depression, or phobia); (c) making the shifts in behaviors, desensitizing conflicts, and acquiring new habits; and (d) stabilizing changes. And the theory of internal drive, ego-psychology, self-psychology, and object relation theory were used to hear the patient's dynamics from the patient, to understand their biological instinct (libido or aggression), and to perceive the defense mechanism and regulate their internal drive. To perceive that the self-esteem and self-value of the patient, summarize the desire, behavior, perception and meaning to the patient of the influence from the important persons, from desire, defense mechanism, self-esteem and the important objects to understand the patient's current mode. Construct conceptualization from the degree of pathology or the developmental level of organization, defense mechanisms, object relational identifications and patterns, and self-esteem. Treated the client sincerely, unconditional paid more attentions to the client's emotions, physical and psychological feelings. Applied the Socrates' question, free association, here and now, emotional reaction and so on to inquire into the source of current symptoms, find a way to deal with anxiety with somatic symptoms with the client. Eased anxiety and improved physical symptoms effectively. The innovations of this paper include: firstly, to provide empirical proof that psychodynamic therapy can effectively ease the anxiety of the client; secondly, to provide some examples of the author's counter-transference and projective identification. The consultation in the paper began at October 2017 and ended at the end of May 2018, with a total of 16 sessions. The author divides the whole consultation process into four stages, exploration stage (1-2nd sessions), comprehension stage (3-6th sessions), consolidation stage (7-11th sessions), and termination stage (12-16th sessions). The client's symptoms related with social anxiety were assessed before and after intervention by Beck Depression Inventory, and results showed a significant decrease in social anxiety after 16-session individual therapy. The client tends to use more mature and more adaptable defense mechanisms. However, the case study does not have universal significance, and I will do more theoretical exploration and intervention research in the future work, hoping to help the intervention of generalized anxiety disorder.
