研究发现,个体的抑郁症状是重性抑郁的重要危险因素,而大学生则是重性抑郁障碍的高发群体。抑郁症状可能会对大学生的情绪、认知和行为方面产生负面影响,一定程度上损坏其社会功能,严重时甚至会引发大学生的自杀意念和自伤行为。通过对大学生的抑郁症状进行提前的识别及预防性干预,能够有效降低高校内重性抑郁障碍和焦虑障碍发病的可能性。目前认知行为疗法和体育锻炼都已经被证明对抑郁症状有良好的干预效果,但很少有研究考察两者结合使用进行心理干预的效果。因此,本研究采用个案研究的方式,在前人研究的基础上,探索二者结合干预抑郁症状的可行性和有效性。在本研究中,咨询师通过筛选和评估,招募了一位存在抑郁症状的研二男大学生,对其进行了认知行为疗法结合体育锻炼的心理干预,包括使用认知行为疗法的10次心理咨询,以及由运动处方指导的为期3个月的体育锻炼干预。干预结束后,咨询师结合量表分数(贝克抑郁量表第2版BDI-II、广泛性焦虑量GAD-7、总体焦虑水平及干扰程度量表OASIS、总体抑郁水平及干扰程度量表ODSIS、罗森博格自尊量表 RES 和自测健康评定量表修订版 SRHMS V1.0)、目标达成情况、咨询师观察和来访者反馈四个方面,对干预效果进行评估。通过效果评估可以看出,来访者的BDI-II、GAD-7、OASIS、ODSIS 评分都有显著的降低,SRHMS V1.0 总分和每个分量表评分都有显著的增加,在回访中也发现其RES的评分有一定的提高。这表明来访者的抑郁情况有较大程度的好转,矫正和调整了一部分负性认知,社会功能有所恢复和增强,大部分抑郁症状都得到了有效的干预。来访者的干预目标基本达成,在情绪、认知、社交和身体健康方面都有较大的改善。同时,咨询师观察和来访者反馈也都反映了本次干预的有效性。本研究的结果表明,认知行为疗法结合体育锻炼对大学生抑郁症状的干预是有效且可行的。本研究可以为后期的相关临床研究,以及新的抑郁症状心理干预方法提供一定的素材和证据。但本研究采用的方法为个案研究,其结果可能具有一定的局限性,具体的作用机制以及更具有推广性的抑郁症状干预方案,未来还需要进一步的探索与研究。 |

It is found that individual depressive symptoms are a crucial risk factor for major depression, particularly in college students who are considered a high-risk group. Preventive interventions for depressive symptoms in college students have proven effective in reducing the likelihood of major depression. Currently, both cognitive-behavioral therapy and physical exercise have been shown to be effective for managing depressive symptoms, but few studies have investigated the effectiveness of combining these two for psychological interventions. Therefore, this research used a case study to explore the effectiveness of combining the two for interventions based on previous studies. In this study, a male college student in his second year with depressive symptoms was recruited for cognitive-behavioral therapy combined with physical exercise psychological intervention, including 10 psychological counseling sessions using cognitive-behavioral therapy and a 3-month physical exercise intervention guided by a sports prescription. At the end of the intervention, the counselor evaluated the effectiveness of the intervention based on 4 aspects: scale scores (Beck Depression Inventory, 7-item Generalised Anxiety Disorder Scale, Overall Anxiety Severity and Impairment Scale、Overall Depression Severity and Impairment Scale、Rosenberg Self-Esteem Scale、Self-rated Health Measurement Scale V1.0), the status of goal achievement, counselor observations, and client feedback. The results indicate that the intervention significantly improved the client’s depressive symptoms. The goals of this intervention were basically achieved, and there were significant improvements in client’s mood, cognition, social and physical health. At the same time, the counselor observations and client feedback also support the effectiveness of this intervention. The results of this study indicate that the combination of cognitive-behavioral therapy and physical exercise is effective and feasible for intervening in depressive symptoms in college students, which can provide some materials and evidence for future works and new psychological intervention methods. However, it should be noted that the current study utilized a case study design, and the results may not be generalizable to other populations or settings. Further research is needed to investigate specific mechanisms underlying the effectiveness of the intervention and to develop more widely applicable intervention programs. |
