研究背景:我国Ⅱ型糖尿病患者人数快速增加,呈现出患病率高、知晓率低、患者对病情的治疗率和控制率低的特点,严重影响居民的身体健康,加重我国公共卫生系统负担。

研究目的:了解我国农村地区Ⅱ型糖尿病患者的健康素养和服药依从性现状,探讨服药依从性和健康素养之间的相关关系,为今后研究Ⅱ型糖尿病患者的服药依从性影响因素以及探寻改善患者服药依从性的方法奠定基础。 研究方法:采用方便抽样的方式选取江苏省盐城市X区内8个行政村作为研究现场,在8个行政村内接受村卫生室管理的糖尿病患者,只要符合纳入排除标准者,均被纳入研究对象。使用一般资料调查表、健康素养量表、服药依从性量表等对Ⅱ型糖尿病患者进行调查,将数据录入SPSS25.0中。通过频率、百分比统计患者一般人口学资料、健康素养水平和服药依从性水平现状,采用T检验和F检验进行服药依从性的单因素分析。最后,采用逐步多元线性回归分析,分析服药依从性的影响因素以及健康素养对服药依从性的影响。 研究结果:Ⅱ型糖尿病患者的健康素养总体得分处于较低水平,平均分为2.62分(SD=0.60),约63.2%的患者处于低健康素养水平,其中功能性健康素养得分最低,平均分为2.42分(SD=0.77),沟通性健康素养水平得分最高,平均分为2.88分(SD=0.76),评判性健康素养平均分为2.54分(SD=0.60)。Ⅱ型糖尿病患者的服药依从性平均分为36.80分(SD=5.14)。逐步多元线性回归分析发现,性别(B=0.530,p>0.05)、年龄(B=0.844,p<0.05)、医生支持度(B=1. 771,p<0.05)、病程(B=0.635,p<0.05)、健康素养(B=2.679,p<0.05)均为服药依从性的影响因素,其中健康素养对服药依从性的影响最大,两者呈正相关。 研究结论:Ⅱ型糖尿病患者的健康素养总体水平较低,健康素养对患者的服药依从性有较大的影响,两者之间呈正相关关系,低水平的健康素养将会导致低水平的服药依从性。因此,社会工作者和医护人员可以从提高患者的健康素养入手,通过提高患者的健康素养,提升患者的服药依从性。尤其是社会工作者,通过干预和提高患者的健康素养,改善患者的自我管理,达到良好的健康结局。 |

Objective: To understand the status quo of health literacy level and medication compliance level of patients with type ⅱ diabetes in rural areas of China, and to explore the correlation between medication compliance and health literacy, so as to lay a foundation for future research on medication compliance of patients with type ⅱ diabetes and find ways to improve medication compliance.
